Information to remember:
Child’s birth certificate documenting the following:
  • For 2022-23
    • Pre K 3 - Must be 3 years old on or before Oct. 1, 2022
    • Pre K 4 - Must be 4 years old on or before Oct. 1, 2022
  • For 2020-21
    • Pre K 3 - Must be 3 years old on or before Oct. 1, 2021
    • Pre K 4 - Must be 4 years old on or before Oct. 1, 2021
Documents needed for registration:
  • Immunization record
  • Health history form (to be completed by parent/guardian. Can be found on the Early Childhood registration web page)
  • Medical examination form (to be completed by a pediatrician or health care provider. Can be found on the Early Childhood registration web page.)
Two (2) current proofs of residency (Please see list below):
  • New Jersey Driver’s License
  • State identification
  • Utility bill (example: PSE&G, SUEZ Water)
  • Telephone bill
  • Cable TV bill
  • Residency evidence on leasing company's letterhead
  • Employment documents
  • Financial account information
  • Benefit statements
  • Notarized letter from private homeowner including names of all persons residing in the residence
  • Unemployment claims
